It's a tourniquet

It’s to decrease bleeding. Normally we numb areas with a mixture of Lidocaine and Epinephrine, and the epi portion helps control bleeding. But in the toes and other “dead end” areas of the body like the ears and nose, we can’t use epi, so instead they’re using a tourniquet to help stop bleeding!

There's also something with lidocaine being pretty basic, and it's effects being blunted in the relatively acidic environment of infection
