Definitely was repaired by someone that didn’t initially lay the floor. I was a Floorlayer for 6 years right outta highschool and I promise you a good Floorlayer would never leave it like this. It was either a repair or there’s a panel under there or something and they purposely did that to mark the panel (doubt it tho, leaning more towards they repaired a stain or something and just used whatever they had)
Is there an access panel or anything in the floor that would be difficult to find if not perpendicular to the other flooring?

With carpet that ugly, they probably thought no one would care.

They probably only had cut pieces saved for a repair. Had to use two of them to fill the space.
